Initial Visit - $495

At Orlando Functional Health, your first visit is intentionally thorough and highly individualized. Unlike traditional symptom-focused appointments, the goal is to understand the complete picture of your health history, lifestyle patterns, previous care, and the factors that may be contributing to ongoing dysfunction or unresolved symptoms.

Your 75-minute, initial consultation includes an in-depth review of:

  • Current health concerns and symptom patterns

  • Past medical and injury history

  • Lifestyle, stress, sleep, movement, and nutrition factors

  • Previous imaging, lab work, or testing (if applicable)

  • Musculoskeletal and functional movement evaluation

  • Systems-based health history and health timeline review

This comprehensive process allows Dr. Colleen to develop a personalized clinical strategy. One focused on improving function, identifying contributing factors, and helping guide the next appropriate steps in care.

Why the Intake Forms Are So Detailed

Your intake paperwork is a critical part of the evaluation process — not “just paperwork.”

The forms are intentionally comprehensive because they help establish patterns, timelines, contributing factors, previous treatment responses, lifestyle influences, and areas that may otherwise be missed during a standard office visit.

Many patients seeking care at Orlando Functional Health have already seen multiple providers or have experienced persistent issues that were never fully addressed. Completing your forms thoroughly allows your case to be reviewed in advance so your visit time can be spent focused on meaningful discussion, evaluation, and planning — rather than collecting basic history information.

Even details that may seem unrelated can provide important clinical context.

Clinic Memberships

Memberships are available following the initial consultation.

Membership Includes, 2× 30 minutes/month or 1×60minute follow up visit/month

Additional visits can be scheduled for a heavily reduced fee

Membership Fee: $225/month

Laboratory Testing & Imaging

Whenever possible, routine blood work and imaging studies will be ordered through insurance-compatible providers. However, some advanced or specialty testing may require out-of-pocket payment.

Many specialized functional tests are not commonly utilized in conventional settings due to limited insurance coverage, cost constraints, or differences in clinical approach. Recommendations for testing will always be discussed in advance, including expected cost and clinical relevance.

Intake Form Requirements

To ensure adequate preparation and the highest quality visit experience, all new patient intake forms must be:

  • Fully completed

  • Submitted at least 24 hours prior to your appointment

  • Accurate and detailed to the best of your ability

Incomplete, rushed, or missing paperwork limits the ability to prepare for your visit and may require your appointment to be rescheduled.

Appointments with incomplete intake forms submitted less than 24 hours prior to the visit may be rescheduled at the discretion of the office.

Multiple rescheduled new patient appointments related to incomplete paperwork may result in a $50 administrative fee.
